WASHINGTON STATE CONSUMER HEALTH DATA PRIVACY POLICY
Effective Date: January 1, 2026
This Consumer Health Data Privacy Policy (“Policy”) applies to Infusion Solutions, Inc collection and processing of consumer health data as defined under the Washington My Health My Data Act (“MHMDA”). This Policy applies to Washington residents and any individual whose consumer health data is collected in Washington State.
This Policy is required by the Washington My Health My Data Act and contains only information required under that law.
I. DEFINITION OF CONSUMER HEALTH DATA
Consumer health data means personal information that is linked or reasonably linkable to a consumer and that identifies the consumer’s past, present, or future physical or mental health status. This includes:
- Information about physical or mental health conditions, status, diseases, or diagnoses
- Information identifying that you are seeking or receiving health care services
- Information about prescriptions, medications, or medical devices
- Reproductive or sexual health information
- Biometric data processed to identify health conditions
- Gender identity information
- Precise location information that could indicate attempts to obtain health services
- Information derived or inferred from non-health information that is used to identify or associate you with health-related information
II. CATEGORIES OF CONSUMER HEALTH DATA COLLECTED
Infusion Solutions, Inc. collects the following categories of consumer health data:

- Health Insurance Companies and Pharmacy Benefit Managers – We share consumer health data to process insurance claims and verify coverage
- Pharmaceutical Manufacturers – We share limited consumer health data for patient assistance programs and specialty medication support programs (only with your consent)
- Shipping and Delivery Services – We share limited information necessary to deliver medications to you
- Legal and Regulatory Authorities – We share consumer health data when required by law or to comply with legal processes
B. Specific Affiliates:
Infusion Solutions, Inc. currently does not share consumer health data with any affiliated entities.
Note: We do not sell consumer health data. We only share consumer health data as described above, with your consent or as necessary to provide services you have requested.
VII. HOW TO EXERCISE YOUR RIGHTS UNDER THE WASHINGTON MY HEALTH MY DATA ACT
Under the MHMDA, you have the right to access your consumer health data and receive a list of all third parties and affiliates — including contact information — who receive your individual data from us. You also have the following rights:
A. Right to Confirm and Access You have the right to confirm whether we collect or share your consumer health data and to access that data.
B. Right to Delete You have the right to delete your consumer health data from our records, including from archives and backups Washington State Legislature. When you request deletion, we will also notify all third parties and affiliates who received your data, and they must honor your deletion request.
C. Right to Withdraw Consent You have the right to withdraw your consent for us to collect, use, or share your consumer health data at any time.

We are required to establish a secure and reliable method for you to exercise your rights
Response Time: We will respond to your request within 45 days of receipt. If we need additional time, we will notify you and explain the reason for the delay.
Appeals: If we deny your request to exercise your rights under the MHMDA, you may appeal our denial by contacting us using the methods above. If your appeal is unsuccessful, you may file a complaint with the Washington State Attorney General at www.atg.wa.gov/file-complaint.
VIII. CONSENT REQUIREMENTS
We will only collect, use, or share your consumer health data:
- With your affirmative consent, OR
- As necessary to provide a service or product you have requested
Before we collect, use, or share additional categories of consumer health data not listed in this Policy, or for purposes not disclosed in this Policy, we will update this Policy and obtain your affirmative consent.
We do not sell consumer health data. If we ever intend to sell consumer health data, we will first obtain your separate, written, and signed authorization as required by law.
IX. UPDATES TO THIS POLICY
We will update this Policy if we collect, use, or share additional categories of consumer health data or use consumer health data for additional purposes. When we make changes, we will update the “Effective Date” at the top of this Policy and obtain your affirmative consent where required by law.
